Insurance
Hometown Pediatric Care accepts most insurance, including Ohio Medicaid, AmeriHealth Caritas Ohio, Caresource, Molina, United Healthcare Community Plan, Buckeye Health, Anthem BlueCross Blue Shield, CIGNA, United Healthcare, Aetna, Medical Mutual, TriCare, plus many of the smaller plans. We are also part of Anthem BlueCross BlueShield’s Enhanced Personal Health Care Program. If you don’t see your company, please call and we will check it for you!
Payment for services
We ask that you bring your insurance card with you to each visit and notify us of any changes in your address, phone number, and/or insurance information.
You will be responsible for payment of charges for services rendered if we are unable to verify benefits with your insurance company. Insurance companies require the collection of your co-pay or contracted percentage of services at every visit. If you do not have your co-pay, we will be happy to bill you, along with a $5.00 service fee.
If you have a deductible that has not yet been met, you will be required to pay for the visit in full. If your insurance company does not pay for a service, the charges will be the responsibility of the parent/guardian. We recommend that you always question your insurance company regarding your benefits first if you have any questions about covered services or bills.
We accept cash, checks, credit card.
Financial arrangements will be required for balances which remain unpaid after 2 statements have been sent, prior to scheduling an appointment.
For those patients that DO NOT have insurance coverage, we will provide a 20% discount on services paid in full at the time of the visit.
There is a $25 fee for returned checks. There is a $25 fee if your account is sent to collections. Active accounts that carry a balance at 60 days after the date of service will be assessed a $5 service charge, to be assessed every month until the account is paid in full.
